ACTIVATE is a digital health equity platform for chronic care coordination. The platform is optimized for implementation in community health organizations with 1) “digital health pathway” models for care coordination in diabetes and hypertension and these models are adaptable for other conditions, 2) implementation toolkit for efficient implementation, 3) digital health literacy resources to help patients embrace technology. ACTIVATE was created by MITRE and UC Berkeley/CITRIS through innovative co-design with community providers and patients and demonstrated real-world health outcomes.

Navigate Maternity Inc.
Navigate Maternity is a tech-enabled medical device company focused on improving maternal health outcomes. Our product is a remote patient monitoring system that captures key biometric data from the first prenatal appointment through twelve months postpartum via the patient’s mobile app and two Bluetooth connected devices: FDA cleared (Class II device) blood pressure cuff and scale. The mobile app delivers validated questionnaires on social determinants of health (SDoH) and peripartum depression. This data allows the provider to intervene earlier on leading postpartum causes of death: hypertensive disorders, cardiomyopathy, bleeding, infection, mental health challenges, and emergent SDoH situations (i.e. domestic violence)

"ThriveLink (formerly known as Nutrible) develops AI social workers, complemented by our human clinical team, that eliminate traditional “referrals” to efficiently enroll patients into housing, food, transportation, ThriveLink’s chronic care management program, and other vital programs with no human interaction from our customers. When families need to speak with someone, we offer human interaction, but more that 80% of medicare and medicaid patients have smartphones, and can independently navigate our easy-to-use platform.
