Collaboration Care is a virtual integrated care program that works with primary care and selected specialty practices to improve the way that adult behavioral health problems (including both psychiatric disorders and risky health behaviors) are managed in this country. This is accomplished through innovative, evidence based scripted counseling content for all of the adult behavioral health problems typically seen by PCPs. This content is reliably provided in a variety of virtual formats (ie telephone or video calls or online, with or without coaching support) by our dynamic cloud based platform and behavioral health coaches. Our programs have been uniquely tailored to meet the specific needs of both PCPs and their patients. They are offered to practices for free, with revenue generated from rarely used billing codes and can be seamlessly integrated into PCPs’ workflows. Our rigorous data collection and analysis supports population health, continuous quality improvement and improved patient outcomes

Health Tequity LLC helps communities build capacity and dismantle barriers to good health through digital health innovation and collaboration. Our unique approach to community co-design leads to digital care coordination solutions that are optimized to community settings and deliver real-world health outcomes. Our flagship product is ACTIVATE, a digital health equity platform for chronic care coordination co-designed with community providers and patients. ACTIVATE is a flexible and HIPAA-compliant cloud platform includes operating system-agnostic remote patient monitoring, dashboards for actionable care coordination, and EHR data integration. The platform is optimized for implementation in community health organizations with 1) “digital health pathway” models for care coordination in diabetes and hypertension and these models are adaptable for other conditions, 2) implementation toolkit for efficient implementation, 3) digital health literacy resources to help patients embrace technology. Our proof of concept is the outcomes we’ve achieved in multiple rural and underserved communities.

Lasa Health

Lasa Health is a Boulder-based health tech startup focused on developing AI and machine learning tools to support clinicians in diagnosing and managing pelvic pain disorders. Chronic pelvic pain (CPP) affects 26% of women and can take 6-12 years to diagnose. Co-founded by Margaret Melville and Chris, Lasa Health aims to improve patient outcomes by reducing diagnostic delays and enhancing treatment pathways for complex conditions like endometriosis and chronic pelvic pain.

The company has received significant support through grants, including from the National Science Foundation and Colorado’s Office of Economic Development, and is collaborating with leading health organizations like the Mayo Clinic. By integrating seamlessly into existing clinical workflows, Lasa Health’s solutions empower healthcare providers to offer more accurate, timely, and personalized care to underserved populations.


Pear Suite is building the best care navigation and billing platform for CHWs. With this platform, CHWs and community-based organizations (CBOs) can better assess and address SDoH challenges, such as food insecurity, housing instability, poor transportation, and limited healthcare access. Pear Suite also enables CHWs and CBOs to contract with health plans and support secure data sharing, collaborative care coordination, and emerging financing mechanisms, such as Medicaid reimbursement, which is about to roll out in Colorado. In addition to building software, Pear Suite has an impressive team of CHWs that help to train and onboard community-based organizations. Pear Suite also serves as the lead agency for smaller community-based organizations who need administrative, clinical, and contracting support; we are currently contracted to serve as a hub for 10 health plans in 3 states.

She Matters, Incorporated

She Matters is a digital health platform transforming Black maternal health through cultural competency and technology. She Matters, founded by Jade Kearney and Marguerite Pierce, offers a unique culturally competent certification for healthcare providers developed with maternal health and equity experts. This certification, the first of its kind, emphasizes patient and provider safety, effective communication, and the history of systemic racism in healthcare. By leveraging technology, She Matters ensures accountability and cultural competence in healthcare. It is a leading authority in Black Maternal Health and a beacon of hope for Black mothers seeking culturally attuned care.


ThriveLink builds telephonic artificially intelligent enrollment agents who talk to families and empower them to verbally enroll in social programs like health insurance, food stamps, and utility assistance with no internet, smartphone, or reading required. Our innovative solution enhances access to vital social services, streamlining the enrollment process to ensure families receive the support they need.


Welfie is on a mission to create generational health and generational wealth for underserved families. We are building a health equity platform for kids that connects youth and their families to youth serving health organizations that include schools, pediatric hospitals, clinics and community based organizations. We focus on increasing health literacy and access to care, ultimately improving the whole child, whole family and whole community health.
